What Is The Bridge Program?

The Bridge Program is a specialized support service that connects patients with the resources, education, and care they need while they continue treatment for a serious illness. It serves as a transition point between active medical treatment and hospice care, ensuring that individuals receive the guidance they need at every step of their journey. The Bridge Program doesn’t provide any medical care or pain symptom management. The purpose of the program is to provide support for the client & their family through various forms of contact and to help with decision making.

This program is ideal for individuals who:
✔️ Have a serious or life-limiting illness but are still receiving treatment.
✔️ Need support managing symptoms such as pain, fatigue, or shortness of breath.
✔️ Are not yet ready to transition to hospice care but want additional emotional and practical support.
✔️ Have had frequent hospital visits or ER stays due to worsening symptoms.
✔️ Need help understanding when hospice might be the right choice.

Unlike traditional hospice care, The Bridge Program allows patients to continue medical treatment while receiving the holistic support they and their families need.
