Patient Care Positions
Our Hospice is looking for Home Health Aides (HHA)/ Certified Nursing Assistants (CNA) for our home side patients. This person works with a caseload of patients alongside a RN Case Manager. Our patients and families are in the most important time of their life and need non-skilled nursing to help navigate their pathway. In this position, every day is a little different and you are on the go visiting patients in your community. This role will work Monday-Friday, no holidays or weekends. The hours are 8 am – 4:30 pm, with some flexibility.

An HHA/CNA at Our Hospice Will:
- Provide routine patient care in the patient’s home or long term care facility per the Home Health Aide assignment in an exemplary manner, providing comfort, care, and compassion to patients and families.
- Assist the patient with daily hygiene activities (including, but not limited to: helping patients to shower, change clothing, shave, and toilet).
- Observe patients and reports observations and concerns to the primary care nurse and/or Patient Care Coordinator, Nurse Facilitator or Manager of Inpatient and Bartholomew.
- Complete patient records accurately in Home Care Home Base (HCHB) and on a timely basis.
- Assist in maintaining a clean, healthful environment for the patient.
- Attend and participates in Interdisciplinary Group meetings, in-services, and staff meetings as requested.
- Participate in a minimum of twelve (12) hours in-service education per year.
- Be willing and able to lift patients for routine hygiene as needed
What an HHA/CNA at Our Hospice Needs:
- High school diploma or general education degree (GED)
- Licensed Home Health Aide (HHA) or Certified Nurse Aide (CNA) in the State of Indiana
- Interest and ability in providing patient care in homes and facilities
- Ability to travel via your own vehicle
- Valid Indiana Driver’s License.
What Our Hospice Provides an HHA/CNA:
- Pay: $16/hr – $17/hr

- Develop and implement the plan of care for each patient in collaboration with the patient, family and Interdisciplinary Group (IDG). Evaluate each patient/family as directed and makes appropriate changes to the plan of care.
- Communicate with and update other IDG team members as needed to coordinate and ensure quality care (e.g. Social Services, Home Health Aides, Volunteers, and Medical Director).
- Promote the Hospice benefit and provides education to all potential referring customers including but not limited to patients and family members, case managers, nursing facilities administrators and staff, physicians and physician office staff, and community groups.
- Maintain accurate records in Home Care Home Base (HCHB) in accordance with the supervising organization’s policies and procedures in a timely manner (e.g. initial plan of care, comprehensive plan of care, and updates to the plan of care, daily documentation requirement, discharge summaries, death summaries).
An RN at Our Hospice Needs:
- Associate or Bachelor Degree in Nursing (ASN/BSN)
- Current Registered Nurse License in Indiana
- One to three years’ experience in health care preferred
- Driver’s License
Our Hospice Provides an RN:
- Pay: starting at $35/hr

Our Hospice is looking for an RN Case Manager – Weekends who will provide on-call service to hospice patients and their families/caregivers after normal business hours in our Columbus, IN service area. This role will be 12 hour shifts on Saturday and Sunday, and on-call 4:30 pm Friday – 8 am Monday.
A RN at Our Hospice Will:
- Provide on-call service to patients/families to ensure availability on a 24 hour, seven day a week basis.
- Triage of incoming phone calls from patients, families, facilities etc. to ensure appropriate hospice measures and provide adequate hospice staff visits that will adhere to a safe and symptom free environment for the patients / families.
- Make routine and on-call hospice nursing visits as assigned to patients in the home, extended care facilities, assisted living facilities, and hospitals.
- Complete assessment of the patient/family needs including physical, emotional, and bereavement status.
- Ensure that hospice care is provided according to regulations and standards established by Our Hospice.
- Manage time, pharmacy and medical supplies, and durable medical equipment in a manner conducive to optimal reimbursement.
- Teach, supervise and counsel patient/family in care techniques including pain and symptom control, dietary instruction, use of equipment and observation techniques.
- Develop and implement the plan of care for each patient in collaboration with the patient, family and Interdisciplinary Group (IDG).
- Evaluate each patient/family as directed and make appropriate changes to the plan of care.
- Communicate with and update other IDG team members as needed to coordinate and ensure quality care (e.g. social services, home health aides, volunteers and medical staff).
- Maintain accurate records in accordance with Our Hospice’s policies and procedures in a timely manner (e.g. initial plan of care, comprehensive plan of care, and updates to the plan of care, daily documentation requirement, discharge summaries, death summaries).
- Collaborate with the Social Worker or Chaplain as after-hours needs are identified for the patients, families or referring facilities.
- Provide support to the Hospice Inpatient Facility as needed in the accomplishment of effective patient / family care.
An RN at Our Hospice Needs:
- Associate’s Degree in Nursing Science (ASN) or Bachelor’s Degree in Nursing Science (BSN)
- One year experience in health care preferred

Our Hospice is looking for a Social Worker for our Columbus, IN region. This person will be supporting hospice patients who are in the end of life phase, as well as supporting their families and caregivers.
This role will be Monday-Friday, with an on-call rotation. A typical shift is 8 am – 4:30 pm, however we are flexible on hours.
A Social Worker at Our Hospice Will:
- Assist in the implementation, development and revisions of the plan of care through the development of problems, intervention, and goals.
- Provide services as indicated by the plan of care.
- Assist patient/family in locating simple legal forms (e.g. Living Will, out of hospital DNR, General Power of Attorney, Health Care Representative).
- Provide psychosocial assessments and psychological counseling for the following but not limited to: a) Patient/family adjustment to terminal illness, b) Individual counseling relating to: stress, anxiety, depression, self-esteem and feeling of inadequacy, c) Family counseling relating to social/emotional problems, d) In the coordination of services for hospice care including: environmental, equipment, temporary care services, long term care and community support agencies, e) Economic counseling to assist in receiving financial aid applications, f) Grief counseling.
- Record in patients chart diagnostically, appropriately, and promptly the social finds, impressions and plan of action in relations to medical social problems.
- Participate in quality assessment and performance improvement program and Hospice sponsored in-service training.
- Participate in Hospice team meetings that serve to coordinator the patient plan of care (IDG) and other team and organizational meetings that contribute to the accomplishment of strategic goals.
What a Social Worker at Our Hospice Needs:
- Bachelors of Social Work (BSW) required, Masters of Social Work (MSW) preferred
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW), Indiana Licensed preferred
- One year of social work experience in a hospital, hospice or home health setting
- Valid Indiana Driver’s License
- Reliable vehicle for home visits

Non-Clinical Positions
- Support the marketing team by managing, editing, printing, and restocking printed materials for the organization while managing digital versions in an organized manner, including creating forms and fillable PDFs
- Manage printer, mail merges, and bulk mailings for the organization
- Support community outreach events and functions including the preparation of materials, managing event supplies, working events, and tracking marketing collateral and outreach activities
- Work independently and within a team on special, nonrecurring, and ongoing projects, including community and fundraising events
- Act as a project manager for special projects at the request of the Director of Marketing, which may include planning and coordinating multiple presentations, disseminating information, and organizing office events.
- Is professional and courteous to internal and external stakeholders
- High School Diploma or GED required; Business school or Associate’s degree preferred; or equivalent combination of education and experience

Skills Needed for a Marketing Administrative Assistant at Our Hospice:
- Proficient in the English language – oral and written

Be responsible for the data entry and gift acknowledgement of all philanthropic excluding events related revenue received.

Prepare notification letters for family members of memorials/honorees.

Add new constituents to the database, maintain and update constituent records regarding any changes in contact information or status.

Perform any and all data entry tasks in accordance with established practices and procedures for Blackbaud/Raiser’s Edge and Our Hospice of South Central Indiana.
